Creekside Community Service Guidelines:
In order to receive community service hours you must be working with a non-profit organization. For county guidelines please click here. Please obtain a letter from the organization you are assisting containing the following information, on letterhead:
- Organization's name
- Organization's address
- Dates of service
- Hours completed
- Supervisor's name and signature
- Telephone number of supervisor
